Run it like a business

It’s that time of year again for the county hospitals to start complaining about budget cuts and threatening to close their doors. I get that times are tough and the amount of people paying taxes decreases every day. What I don’t understand, is how organizations that bill for their services, in addition to receiving subsidies can be doing so bad?

The first complaint I typically hear is; “We get all the bums, drunks and non-paying patients”. I absolutely agree. However, there is no reason why they should be the place that all the people with no insurance go. They are fully functional and capable hospitals just like the private-based hospitals in the same area, who don’t receive subsidies.

Why don’t these hospitals adopt specialties, advertise, secure HMO contracts, and expand, just like every other hospital? The mentality that only people without insurance go to county hospitals is ridiculous. They need to be in competition to increase their revenue. Start convincing the 58 year-old with chest pain and insurance that the county hospital is the best place to go.

Perhaps I’m just a dumbass, narrow-minded, paramedic who has no idea what he’s talking about, but doesn’t it seem reasonable that if these hospitals ran themselves like a business, that they might succeed like a business? Couldn’t this make a difference in the cost of healthcare all together? Any insight to this matter would be much appreciated.


Abuse – behind the scenes look at healthcare costs

Everyone seems to be talking about how we can fix our “broken” healthcare system. Yet, nobody seems to talk about why healthcare is so expensive in the United States. You can blame it on insurance or the government. But what it really all comes down to is abuse and law suites.

Abuse and law suites are like siblings when it comes to health care. Massive amounts of people abuse our emergency rooms, ambulance services and even clinics. This is allowed to happen because these people can’t be denied medical care as the medical providers will find themselves in legal trouble.

Of course the people abusing our medical system typically have government aid, or don’t pay at all. Medicaid and Medicare programs are only reimbursing 20-30 cents on the dollar right now. That’s pretty bad considering what it costs to do business in healthcare. So what do the providers do to make up this lost revenue? They raise their prices! Which ultimately means that the private insurance companies get shafted with the high rates. When people abuse the system and don’t pay their bills, the healthcare providers lose money on supplies, wages, procedures, etc. It’s no wonder that the majority of private doctors offices and clinics don’t accept government insurance programs. Why would they?

Law suites closely follow abuse on the “rising healthcare costs” scale. Excessive medical malpractice suites have driven malpractice insurance through the roof. Once again, the doctors have to make up this loss somewhere. It is actually rare to find a doctor who has yet to have a malpractice suite filled against them. Whether the suit is legit or not, it is typically easier and cheaper for the insurance companies to just settle and pay up rather than fight it out in court. Excessive malpractice suites then lead to “defensive medicine”. Ordering expensive unnecessary tests to “cover their asses”. Once again, only contributing to rising medical bills. Doctors wont follow their training and instincts in fear of legal repercussions.

Does this mean that a doctor who shows gross negligence shouldn’t be held liable for their actions? Absolutely not. But suing your OB/GYN because your baby came out with 6 fingers isn’t what I would call reasonable.

Throwing more money at the problem doesn’t fix anything. We need to attack this problem from the ground up. Hospitals need to be able to deny treatment to non-urgent patients. They crowd our emergency rooms and eat up useful resources. Perhaps some public education and readily available low-cost clinics and urgent cares would tackle a good portion of this problem. The other thing that needs to be addressed is the idea that being transported by ambulance will get you seen faster. One of the big ideas behind EMS 2.0 is that EMS professionals could offer other solutions, rather than just transport to an emergency room. These are just a couple things that could significantly reduce the cost of healthcare.

Let’s quit covering up these problems and attack the real issues. Think of it like treating an illness. You can treat the symptoms and give some temporary relief. Or you can correct the core of the problem and fix it all together.


If Barack Obama was a Paramedic

In light of the recent passing of “healthcare reform”, I felt that this weeks Celebrity Medic should feature our Commander in Chief. Today we will discuss what happens when you take his ideas for improving healthcare and applying it directly to the EMS setting. So of course, we will be answering the big question….

What kind of paramedic would Barack Obama be?

Shifts

Barack Obama would most likely be on a part-time schedule as he is far to busy traveling outside of the country to be tied town to a regular shift.

Scene Safety

We all know the president has the luxury of the Secret Service. When Barack Obama is dispatched to a call, it would be hours before he could safely arrive on scene as the area would have to be secured by snipers, road blocks, check points, etc.

Response

Barack Obama would only respond to calls that he has been prepped to handle. The patients would be carefully selected prior to dispatch to ensure that they didn’t present with an illness or injury that he wasn’t prepared to care for.

Vehicle

Air Force One would most likely be retrofitted to act as a mobile hospital. This might sound expensive and you might wonder how he would fit in into his operating budget. This would simply be achieved by placing a “you call we haul” tax on all ambulance rides around the nation.

Radio Reports

The radio is old and out of date. Barack Obama would hand off his patient care information in the form of town hall meetings and addressing the public via TV and Twitter.

Partner

Obviously Joe Biden would be Barry’s right hand man. Only he would be “that partner” that says dumb things on scene and gets you into trouble with your supervisor. This wouldn’t matter as they really don’t have to answer to anyone. My guess is Joe would be an EMT-Basic.

Billing

Patients treated and transported by Barack Obama wouldn’t have to worry about payment as only those who don’t use or want his service would be billed.

Equipment

Barack Obama wouldn’t carry much equipment as his practice is based on hope rather than experience, training and expertise.

  • Microphone – For addressing his patients and for use during his radio reports.
  • Teleprompter – Used to scroll pertinent questions for his patient assessments.
  • “The Briefcase” – Instead of being using to launch nuclear weapons, it will now be used to summon the real medical professionals behind his operation in the event of a real medical emergency.

Marriages and Heart Blocks

The title will make sense soon……I promise.

I figured I would share with you all something I made up while I was in paramedic school learning ECGs. It’s an easy way to memorize the different heart blocks and how to recognize them. It all revolves around a story about a couple named “P” and “QRS” (clever huh?). Different stages of their relationship will represent the blocks.

Normal Sinus

P and QRS are in the honeymoon phase of their marriage. They are always together and never leave each others sight. Life is good for P and QRS!

First Degree AV Block

P and QRS are getting irritated with each other. While they still remain true to the relationship, they are keeping their distance for now.

Second Degree AV Block (Type I)

P and QRS have entered the viscous cycle of breaking up and getting back together. They get together, they start fighting and keeping their distance, and they break up again.

Second Degree AV Block (Type II)

P and QRS have decided to get back together again. However, P is still quite disgruntled and has decided to go out every night or two and fool around on QRS. You can probably imagine what comes next!

Third Degree AV Block

As you probably saw coming, QRS caught P cheating and moved out. They have filed for divorce and are completely separated. There is absolutely no relationship between P and QRS now.

If Chuck Norris was a Paramedic

If you have never heard of Chuck Norris, then you have been living under a rock with no daytime cable. From the “Delta Force” to “Walker Texas Ranger”, he has shown the world that he is one certified badass. Now today we are going to talk about what happens when you take Chuck Norris from the role of kicking ass to saving lives. So now the big question……

What kind of Paramedic would Chuck Norris be?

Shifts

Chuck Norris doesn’t work shifts. He tells people when they are allowed to have emergencies. This is done around his busy schedule. At no time will he be tied down to scheduled hours.

Scene Safety

We all know the “Texas Ranger” doesn’t need to cleared to a scene. As a matter of fact, he prefers to live life on the edge. The more dangerous the scene, the better. My guess is law enforcement wouldn’t be dispatched to any of his calls.

Response

Chuck Norris doesn’t respond to calls. The calls respond to him. When he gives the OK to have an emergency, patients will make their way to his location. He can’t be bothered with driving and trying to find peoples homes.

Vehicle

Chuck Norris doesn’t need a vehicle to respond in, as he doesn’t respond. Patient’s seeking his services must provide their own form of transportation.

Partner

Come on now…..we all know Chuck Norris works alone!

Equipment

Chuck Norris doesn’t need medical gear, tools or medications. Disease processes quiver at the very sight of Chuck Norris and have no choice but to immediately comply to his demands. Patients suffering from trauma usually got their injuries as a result of a Chuck Norris beating. There isn’t much treatment that can be done to improve their condition anyway.

Oh, so do you drive?

Ever heard that phrase? If you work in EMS then you probably hear it every time to tell someone that you are an EMT or Paramedic. If you are anything like me, then you probably used to respond by explaining exactly what you do. After answering that question enough times, you have probably broken down and just say “yep” with hopes that the conversation doesn’t go the “what’s the worst thing you have ever seen?” route.

To this day, I am absolutely amazed at how everyone in the world seems to be completely clueless as to what we as EMS professionals do. It’s so bad that people who have been transported by ambulance still think that we just drive. While getting my hair cut a little while back, the lady cutting my hair told me that she had been transported to the hospital twice. She then went on to ask if we “just drive the ambulance”. Of course I asked her if the crew just showed up and gave her a ride, or if they treated her injuries.

Perhaps TV is to blame for this problem. It’s rare to flip through the channels and not see a show about cops. Shows and movies about firefighting are not as common, but still out there.  Shows that focus on EMS are far and few between and they typically flop. Hollywood hasn’t exactly done us any favors either. Sure, “Mother, Jugs and Speed” was a hit, but what else do we have? “Bringing out the Dead” just made us look like a bunch of burned out, delusional drug addicts. OK……maybe not far from the truth.

So how do we improve public awareness? I would hate to think that the only way to gain respect from the public is to make a TV drama series. I really wish I could end this post with some intelligent solution to the problem. Instead I’m asking for ideas. We really need to make the general public aware of what we do. We provide a valuable service to the community that deserves recognition. But more importantly, we need to make the elected officials that are making decisions truly aware of what we do. Perhaps doing so might change the way our legislators think when it comes time to cut funding for programs and reimbursement.

And that's why I do this job

I just finished reading a great post on Jeramedic’s Blog about his first true save in the field. The whole time I was reading it, I couldn’t help but remember my first. Fortunately Jeremiah got to see the save right before his eyes. I had gotten pulses back in the field, but I knew (or thought) the downtime was to long for her to have any real neuro activity. I actually left the hospital sure that she wasn’t going to make it. That’s where this story gets good.

I was teaching an EMT class at the local college one night. The semester had just started and I having the students introduce themselves and explain why they want to become an EMT. After 20 minutes of the usual “I need this for my job” or “I want to be a firefighter”, a student asked to share a story if I didn’t mind. I encouraged him to take all the time he needs.

He walked up front and told the story of how he witnessed his mom collapse and go into cardiac arrest. He talked about how he didn’t know what to do and how scared he was. Then he started to explain what happened once the ambulance showed up. As his story went on, I started paying closer attention. This sounded familiar…..very familiar. After he told his story, I excused myself from the class for a minute and called a break. I made a call to my dispatch center and asked to reference the last name and see if any patient’s were run for a cardiac arrest in the last year. Just as I suspected, I ran his mom a little over a year ago. I was the paramedic that he was talking about.

Not quite sure how to approach this, I asked to speak to him in private. I thanked him for sharing the story and broke the news. He immediately teared up and gave me a big hug and thanked me over and over again. He went back in the class and announced to everyone that I was paramedic that saved his moms life. Everyone clapped for a good few minutes. I was full of emotion and didn’t really know how to act at that point. He went on to tell everyone that this is the very reason he wanted to become an EMT and eventually become a paramedic. I couldn’t believe it. I had inspired someone to do this job without even knowing.

He asked if I would be willing to share my side of the story, to which I did. I started from the point where we were dispatched an explained the series of events up until we reached the hospital. I watched tears come to everyones eyes and it wasn’t until that point that I truly understood why I do this job.


If Batman was a Paramedic

The other day I noticed a paramedic from out of the area with quite a large “utility belt”. Of course I nudged my partner and said “that guy has more gadgets on his belt than Batman”. Now we all know that such a thing isn’t really possible as NOBODY can outdo Batman. This did make me wonder what Batman would carry if he was a career Paramedic? Better yet, what kind of Paramedic would he be?

Night Shifts

“The Dark Knight” doesn’t do days. My guess is he would probably work a 19:00 to 07:00 shift. This would pretty much guarantee night time hours.

Scene Safety

Batman has never heard of the word “staging”. As a matter of fact, he clears the scene to make it safe for the cops to enter. You can be assured that he carries all his personal protective equipment at all times.

Dispatch

I think we all know how Batman would be dispatched to calls. The Bat Signal! This of course would have to be accompanied by some form of on-board computer in his rig. Even Batman needs some way to keep track of times and information for his paperwork.

Vehicle

The Bat-mobile is great for first-responding, but not so great for transporting patients. My guess is that he wouldn’t work on the ground at all. He would probably retrofit the Bat-Copter to serve as an air-ambulance. Of course, Batman’s night vision capabilities would allow him to handle more high-risk flights. Now some of you might ask “how is Batman going to fly and do patient care and the same time?”……Come on guys, He’s Batman!

Partner

Every good Paramedic needs a partner. Of course we all know that Robin is the man for the job. Only, I don’t see Robin as being a Paramedic. I see him as a really good EMT-Basic or Intermediate at best.

Equipment

OK, now comes the good stuff. I am going to list some essential equipment that Batman would use.

  • Bat Sheers – Needed for cutting clothes, leather, car doors and seat belts.
  • Bat Stethoscope – You think your “Cardiology 3″ is good? You can put his to the ground and hear a heartbeat in China!
  • Bat Radio – Consists of a tiny chip placed in his mask. No pesky Motorola’s for Batman!
  • Bat Scope – A laryngoscope made of carbon fiber. It also has unlimited battery life.
  • Bat Light – A penlight with x-ray vision.
  • Bat Monitor – A pair of cool-looking glasses so he can just at the heart right through the chest.
  • Bat Computer – 1,000 times tougher than the Panasonic Toughbook.
  • Bat Phone - Integrated with the Bat-Radio chip in his mask.
  • Bat Board – A backboard that folds into a pocket-size. My guess is that it would be made of some form of steel.

These are only a few examples of what could possibly be in Batman’s inventory. I am now calling on each of you to help me list the essential tools for Batman to carry while he’s on-duty!
